PRE-APPROVAL REQUIREMENTS for BANK FINANCING:

- 1×1 6 pieces photo I.D.

For Locally Employed
- Latest Income Tax and/or BIR Form 2316
- Certificate of Employment with Salary
- Latest Full Month Payslip
For OFW/OCW
- Latest Crew Contract (for seaman)
- Employment Contract duly authenticated by Philippine Consulate (for nurse and contract workers)
- Six(6) months Latest payslips (updated)
- Proof of remittance or allotment slips for the last six(6) months (updated)
- ITR
For Self-Employed/Professionals
- Income Tax Return for the last two(2) years
- Audited Financial Statements for the past two(2) years
- In-House Financial Statements for the last two(2) years
- Certificate of Business Registration of w/ DTI
- Statement of Account or photocopy of passbook for the last six(6) months
- List of Major Suppliers and Customers with contract information (at least 3)
- Business Background (if applicable)
- Clinic permit (for Doctors)
- Clinic Addresses and Schedule
- List of Accredited Hospitals
For Pensioner
- True copies of pension remittance slips and/or notices &/or passbook/bank statement of account where pension is Directly Deposited
If Corporation
Any of the following:
- Copy of SEC Registration/Articles of Incorporation/ By-Laws (for corporate business)

For Home Improvement/House Construction

- Building Specification Certified by an Architect/Civil Engineer

- SPA – duly notarized by the Philippine Consul where the applicant is currently assigned (using BDO Format)
